Evidence-Based Practice in Nursing is needed for an effective, safe and efficient health care. The evidence-based practice can be initiated by realigning education and curriculum, also doing research in the new fields, as well as theory and model development. For better patient care newly acquired knowledge must be changed to useful forms and it should be implemented throughout the system and evaluate the effectiveness. The quality is depended on three aspects. They are intervention, targeted outcome, and consistency with current knowledge. The EBP is to implement our fullest knowledge what we do in patient care.
In our facility standard of practice protocol for early recognition and appropriate management of individuals with dementia is implemented as an evidence-based nursing. First assessment of cognitive parameters by using mental status screening like, clock drawing test, mini-cognitive etc. Second functional assessment followed by monitoring behavioral and depression evaluation, physical examination, evaluation of medication, nutrition, monitoring the effectiveness and side effects of medication. As cognitive-enhancement techniques, adequate rest, sleep and pain control are monitored. Avoiding physical and chemical restrain and maximize functional capacity was another change. Education and support for caregivers also a part of the plan. Follow up is done by giving respect the space, distract, redirect, also ensuring therapeutic and safe environment. Also identified and rectified the environmental triggers, medical conditions and care giver-patient conflict. By doing this minimized the distress symptoms. Effective care giving skills give less burden and higher satisfaction for care givers. The institution benefited providing a safe environment for delivering care with high quality.
