The Institute of Medicine (IOM), established in 1970 was created to inform the nation on healthcare issues. The reports and findings of this group assist the divisions of healthcare in making knowledgeable decisions. According to the IOM report, less than 1% of nurses hold Doctorate degrees. In recent years more Doctorate programs have been added to the Graduate curriculum. This is a vital role in nursing education as the Doctorate prepared nurse is needed to provide teaching to future nurses, and contribute to research to improve nursing practice. Through expanded education, comes leadership and leadership roles. Nurses must be accountable and collaborate with leaders from various healthcare professionals to improve quality and reduce common errors, to community leaders to improve health care reform.

Nurses are poised to assume greater roles and responsibility through the primary care setting. Advance Practice Nurses care for patients from pediatric to geriatric; managing diseases, chronic medical conditions such as diabetes and hypertension to acute illnesses. However, barriers exist in the healthcare system that does not allow Advance Practice Nurses to practice to the fullest extent of their abilities. Regulatory barriers is one of the main issues. Some states will allow Advance Practice Nurses to work in their scope of practice, but some do not. The IOM recommends to “expand the Medicare program to include coverage of Advance Practice Nurse services that are within the scope of practice under applicable state law” (IOM, 2010).
