Six Dimensions of Health Worksheet
For each of the following six dimensions of health, list at least one characteristic, activity, belief, or attitude that reflects that dimension in your life. Provide a brief explanation with each example. Refer to Ch. 1 in the text for explanations of these dimensions.
Physical health: Exercise at least five days a week. Physical fitness has to be up to par to stay in the military. I usually watch my diet because I am not as young as I use to be.
Social health: Right now I really do not have a social life. It might not be healthy but I guess I do it to protect myself. Having to move around a lot does not allow me to build the friendships that I am looking for.

Once my son was born, I knew the life had to change. I decided to work on bettering myself to be a better role model for my son. To that, I needed people who will not derail me into another direction.
Emotional health: My wife is someone who needs to hear “I love you” most of the time. She does admit that I choose the perfect times to tell her that I love her. I finally convinced her that if I say it all the time, it will become repetitive and eventually have no meaning anymore.
Spiritual health My belief in life is being positive about everything. Having positive thinking will attract positive outcomes. If I say I can do it, it will happen. It still takes some work but I will continue to build myself to be positive as much as I can.
In approximately 125 to 200 words, describe health and wellness in your own words using the ideas and concepts for each of the six dimensions of health.3
For someone to be completely healthy, you have to be healthy physically. Being healthy physically means you are physically fit and not have any kind of disease or sickness. To be completely healthy you also have to look how you feel emotionally. People usually express themselves the way that they feel emotionally, if there are feeling upset that is the way they are going to react to any situation or start of conversation. Knowing how to control how you express yourself regardless how you feeling are a way to understand how your emotional health is. Spiritual health is also an important aspect of your health and well-being. Believe in something that is bigger than you lead you to try and reach that high being. It also gives you goals. Your intellectual health allows you to learn from your mistakes and success to become the person your will eventually become. Environment health is simply what you surround your life with. Social health is having friends and family or someone to have fun with, speak about your problems to, and listen to someone else problems.
